This form is for Ultra Residential Gas Boilers only.

To make a claim on any other product please use the General RGA form.

This form must be completed in full.

  1. Stating “defective” is not a sufficient reason for return. Please describe the nature of the defect.
  2. CP number is mandatory. If necessary, call customer service for assistance.

If all required information is provided and/or digital photos have been emailed, replacement part(s) will be shipped out immediately to the address indicated below.

The following terms and conditions apply:

  • Part will be no charge if the boiler is less than 5 years old.
  • Shipping is the responsibility of the end user. The part will be shipped prepaid and charge.
  • The defective part is not required to be returned.
